This agreement and the policies established herein are between the lessons studio, Elliott School of Music (ESOM) and the customer/student. All teachers are independent contractors and are solely responsible for the instruction of regularly recurring lessons. All other matters of business are solely between ESOM and the customer/student.
2. BILLING.
ESOM bills monthly at $130 per month. ESOM may change this rate with no less than 30 days notice. There is a one time $25 registration and the first month is prorated. By providing card information to ESOM, you are authorizing payment to be drafted from the card provided on the 1st of each month that you are enrolled for lessons. This authorization is valid indefinitely, unless you close your lessons account via a 2 week stop notice to ESOM.
3. MAKEUPS.
In the event that a student doesnât attend a lesson or provides cancelation notice within 24 hours of the lesson, there will be no make up or credit for the missed class. If the student provides the teacher with more than 24 notice, the class may be made up before or after the absence. Any lessons missed due to inclement weather, teacher absence, or holiday closures will be made up before or after the missed lesson.
4. SUBSTITUTES.
In the event that a teacher is unable to schedule makeup classes, a background-checked, authorized substitute teacher may be scheduled to teach in the place of your usual teacher.
5. CANCELLATION. To stop lessons, a 2 week stop notice is required. Final payment is due at the time of stop notice. All outstanding make-up lessons are forfeited once a 2 weeks notice is given to the teacher or ESOM management.
6. LATE PAYMENT.
Payment is due and drafted from the customers card on file on the 1st of each month enrolled. Should a studentâs payment not be made on the 1st of the month, ESOM management will contact the student with a reminder that timely payment is required to continue. If payment is in excess of 7 days late, ESOM reserves the right to cancel enrollment in the program(s) and allocate the studentâs time slot to another student.

Elliott School of Music has completed background checks on all teachers and we regard the safety of students as our top priority. As such, all lessons are video and audio recorded for both safety and quality assurance purposes and any lesson may be attended by parents and/or guardians.
